EMPLOYMENT FORMS New hires will need to complete the appropriate student Employment Forms
listed below. You will need Adobe Acrobat Reader 4.0 to view and print the
Portable Document Format (PDF) files. Click Click on INSTRUCTIONS for information on filling out PDF forms online. Student worker's Employment Forms-Refer to the appropriate column and follow the steps (example: a new hire international student worker will complete steps 1-7 in that column). Remember to bring 2 forms of ID.
Please read the instructions carefully. Record of Employment form should be complete for each student worker in your department. Also complete this form if you make any changes in your student employment i.e. from Regular Campus to Work Study student worker or from one department to another. I-9 Form & I-9 Instructions - Employment Eligibility. Complete highlighted information. W-4 form - Employee’s Withholding Allowance Certificate. Complete highlighted information. Workers’ Compensation Insurance Notice is provided to all person employed with Texas A&M University-Commerce in accordance with Chapter 502 of the Texas Labor Code. Selective
Service Registration Status - For all Males ages 18 - 25. Complete
highlighted information.

on-campus student workers are required to complete the Employment Discrimination
Training. This self paced module. After completion, the verification
form must be signed and forwarded to the Student Employment Coordinator in the
