Purpose of Form
-
The purpose of this form is to aid the Accountable Property Officer (APO)
in managing equipment and to protect A&M - Commerce against theft or
loss of this equipment.
Submission of Form
-
The APO of each department should complete this form before submitting a
copy to the Property Manager. The original of this document should be kept
by the APO. A copy of the agreement should be forwarded to Property
Management Services, Attn: Property Manager.
A copy of the confirmation of property
being returned should be submitted to Property Management Services.
Completing the Form
-
Complete all three sections of the form. Section one contains information
found on the equipment information sheet. Sections II and III need to be
completely filled out by the APO also. Additionally, signatures of both
the borrower and the APO (department head) are required.
Responsibility of the APO -
The APO must make sure all information on the form is correct. Forms need
to be updated (and the Property Manager informed) when changes occur.
