Purpose of Form -
The purpose of this form is to cancel a previously submitted Missing or
Stolen Property Report (INV908).
Submission of Form
-
The Accountable Property Officer (APO) for each department should fill out
and submit this form to the Property Manager when property that was
reported as missing or stolen is found. Each department should
retain a copy for your records.
Completing the Form
-
The department, asset number and asset description should be filled in. It
is also very important to fill in the building and room number. Please be
sure to sign the form prior to submission. This information will be used
to update FAMIS and to notify the State Auditor's Office through the State
Property Accounting system.
Responsibility of APO
-
It is important that a cancellation is submitted and processed in a timely
manner. Please verify the accuracy of the asset record and note any
changes that need to be made upon returning this asset to an
"available" status.
