Missing, Damaged or Stolen Property Report (INV908)

Purpose of Form -
The purpose of this form is to notify the Property Manager of missing or stolen equipment. It is to be filled out as soon as an item in inventory is discovered to be missing or stolen.

Submission of Form -
The form should be signed by the department Accountable Property Officer (APO) and submitted to the Property Manager. Each department should retain a copy for your records.

Missing Property: Within 24 hours the APO must notify the Property Manager of the missing equipment. Within the same 24 hour period, a notice should circulate around the department in case someone else in the department has any information about the missing equipment.

Stolen Property: The APO should report the incident to the police department within 48 hours of identifying the theft. The original police report, along with this completed form, should be sent to the Property Manager for further action to be taken.

Completing the Form -
Include asset description, asset inventory number, asset serial number(s), place of occurrence, last known location, and APO signature. Also attach correspondence from the department head documenting that steps 1-9 listed on the form were taken. Leave the bottom to be filled out by the Property Manager.

Responsibility of APO -
The APO is in charge of making sure all information on the Missing or Stolen Property Report is correct and that all steps were taken to find the missing or stolen item.
